Request a Certificate of Insurance
If you would like to request that a Certificate of Insurance to be issued to a third party, please fax a written request to 212-576-9701. The request must include the signed authorization of the insured or policy administrator, as well as the name, address and, if applicable, the fax number of the certificate holder.
