Today I'm going to show you how to use MLMIC's new rapid application for new dentists.
So, starting here on the MLMIC homepage, MLMIC.com, you'll click over to dentists, click apply now, and you'll see right here specifically for new dentists, it says Apply online using our five-minute application.
So, once you open that up, you will put in your email address, twice. You'll put in your last name, and you will put in your license number. And now this will open up the beginning of the rapid application.
And again, this should take you five minutes less than the time it takes to fill a cavity.
These preliminary questions are just three yes or no questions that will open up the general information. This is your first and last name, email address, mailing address, that basic information. Once you fill that out, you'll go right on into education. And if you don't complete anything, there will be a popup window that reminds you to go back and finish that up, as well as this red exclamation point. Under education, you have the chance to pick your school. There's also a dropdown for degree and to put information about when you graduated and your license information.
When you move on to practice, you will fill out information about where you will be practicing. The office, whether or not you're a member of NYSDA – we encourage you to join NYSDA if you're not a member already – and information about your primary practice specialty. So, you can fill that out in this dropdown right there.
Moving on to underwriting, this is just three quick yes or no questions that you will fill out right here. On the claims page, you won't have to do anything here, this page is intentionally left blank.
Finally, onto authorization. You will just confirm that you've reviewed the information that's accurate. You'll put your initials right here, and then you'll click submit. And you'll hear back shortly from someone from MLMIC who will confirm whether or not you've been approved.
